SPECIAL SYMPOSIUM: Pioneering an Effective Stroke Rehabilitation Paradigm During the Acute Care Hospitalization ACRM Despite advances within segments of the stroke continuum of care, seamless transitions remain a challenge. To address this, the Stroke Rehabilitation Service Line (SRSL) was created at UT Southwestern Medical Center to coordinate care by integrating acute stroke and post-acute rehabilitation services. This ACRM Conference Special Symposium discusses development and implementation of the SRSL, including identification and recruitment of key stakeholders, establishment of key outcome measures, and selection of essential strategies to sustain stakeholder participation.
